If you are medically homebound, please call (478) 738-3240 to request assistance during office hours of 8 am to 4 pm. Homebound is defined as someone who cannot leave their home due to medical or physical limitations.
Required Documents for appointments (digital copies will not be accepted):
- Valid Picture ID
- Social Security cards for all household members
- Proof of income for the past 30 days
- Current gas & electric bills (must be printed)
**Households with a bill credit of $1,000 or more are ineligible for LIHEAP benefits.**
